TERMITE CONTROL CONTRACT
THIS CONTRACT is entered into between the CLEMENTS' PEST CONTROL & GARDEN SUPPLY of
Leesburg. Florida a licensed "TOX-EOL" contractor and
City of Clermont, the owner or real estate agent of the building located at
Clermont, Florida
THE TERMS OF THIS CONTRACT ARE AS FOLLOWS:
1. The contractor agrees to treat the above described building with the "TOX-EOL SYSTEM" of termite control
which is in accord with the latest technical methods and the instructions of the chemical manufacturer.
2. The contractor agrees to render such further treatments as are necessary to completely eradicate subterranean
termites during the duration of this contract.
3. The contractor agrees to remove all debris, clay tunnels, and trash from under the above described residence.
4. REPAIR WORK
5. On completion of initial treatment the owner agrees topay the contractor $80.00*
as follows: *($90.00 less discount of $10.00., balance $80.00)
The contractor agrees. to make a thorough re-inspection within six months after treatment and annually thereafter
and take the necessary measure to eradicate any termites should re-infestation be found for 4 consecutive
years for an annual fee of $ 10. 00 , payable when re-inspections and re-treatments are made.
6. CITY OF CLERMONT agrees not to store wood,paper or other cellulose material which might
furnish food for termites under the house, and to repair promptly any plumbing leaks or other conditions arising after
the execution of this contract, which might cause the collection of moisture under the house. ;" :
7. It is agreed between the parties to this contract that the prices quoted herein are subject to change in the dis-
cretion of the contractor if he is to render these services to any additions made to the above described building after the
execution of this contract.
8. It is understood and agreed between the parties to this contract that this contract embodies the entire agreement
between the parties and no covenant, agreement or representation, oral or written, expressed, ,or implied, shall limit, modi-
fy or qualify the terms of this contract and its provisions.
9. It is agreed between the parties that this contract is not assignable, but in case of sale of the above described
premises, the purchaser shall have the privilege of entering a new contract for the unexpired terms of this contract at
the same rate.
10. This contract does not cover powder post beetles.

THE TERMS OF THIS CONTRACT ARE AS FOLLOWS:
1. The contractor agrees to treat the above described building with the "TOX-EOL SYSTEM" of termite control
which is in accord with the latest technical methods and the instructions of the chemical manufacturer.
, 2. The contractor agrees to ,render such further treatments as arc necessary to completely eradicate subterranean
termites during the duration of this contract
3. The contractor agrees to remove all debris, clay tunnels, and trash from under the above described residence.
4. REPAIR WORK
5. On completion of initial treatment the owner agrees to pay the contractor
$65.00* as follows:
(*75.00 less $10.00 discount, balance $65.00)
The contractor agrees to make a thorough re-inspection within six months after treatment and annually thereafter
and take the necessary measures to eradicate any termites should re-infestation be found for 4 consecutive
years for an annual fee of $ 5.OO , payable when re-inspections and re-treatments are made.
6. CITY OF CLERMONT agrees not to store wood, paper or other cellulose material which might
furnish food for termites under the house, and to repair promptly any plumbing leaks or other conditions arising after
the execution of this contract, which might cause the collection of moisture under the house.
7. It is agreed between the parties to this contract that the prices quoted herein are subject to change in the dis-
cretion of the contractor if he is to render these services to any additions made to the above described building after the
execution of this contract.
8. It is understood and agreed between the parties to this contract that this contract embodies the entire agreement
between the parties and no covenant, agreement or representation, oral or written, expressed or implied, shall limit, modi-
fy or qualify the terms of this contract and its provisions.
9. It is agreed between the parties that this contract is not assignable, but in case of sale of the above described
premises, the purchaser shall have the privilege of entering a new contract for the unexpired terms of this contract at
the same rate. ' ,
10. This contract does not cover powder post beetles.

The chemicals used in the Tox-EOL SYSTEM OF Termite Extermination are guaranteed to be highly toxic
to subterranean termites and other wood eating insects and to be entirely effective for the control of such pests
when applied according to the manufacturer's instructions by a capable operator.
If an inspection by the Licensed Agent should reveal re-infestation by subterranean termites in the timbers
treated within five years from date of the original application then the manufacturer will furnish the necessary
chemical and Licensed Agent the required labor for such re-treatment as may be necessary to control such
re-infestation without cost to the holder of this Warranty. The Licensed Agent will make regular periodical
inspections of the property treated during the life of this Warranty.
This warranty is limited to applications made by a licensed Agent of the
manufacturer and to the property treated as shown by the inspection report attached to
and made a part of this Warranty.
