CHANGES ORDERED:
I. GENERAL This change order is necessary to cover changes in the work to be
performed under this Contract. The General Conditions, Supplementary Conditions,
Specifications and all parts of the Project Manual listed in Article 1, Definitions, of the
General Conditions apply to and govern all work under this change order.
Change Order No. 1
II. REQUIRED CHANGES: Extend the contract date by 268 days and the line item costs
to remobilize the drill ng to the Disston Avenue-potable supply well site,prepare
the site to resume pilot hole drilling from 550 feet to 1,650 feet bls(est 1,100 feet).
Pilot hole drilling would include collection of lithologic samples and drill stem
water quality samples to track water quality with depth into the productive portion
of the Lower Floridan aquifer(LFA). Once pilot hole drilling has been completed
then conduct geophysical and video logging to determine LFA production zone, and
conduct up to two (2) dual element packer tests to isolate the productive portions of
the LFA and collect water quality samples to confirm water quality.
III. JUSTIFICATION: High TOC water in the Upper.Floridan aquifer has increased
the anticipated water treatment requirements and increased the anticipated costs to
treat water from the Disston Avenue potable well. Increasing the depth of the pilot
hole from 550 feet bls to approximately 1,650 feet bls would allow for collection of
water samples from the productive portion of the LFA,to determine if suitable
water quality exists at this location in the LFA to modify the Disston Avenue potable
supply well and reduce the water treatment costs.
IV. PAYMENT: The attached schedule of values is provided as Exhibit A and shows a
credit of$67,438.00 from the existing contract after all work was completed. The
attached change order estimate is provided as Exhibit B and shows the total estimated
costs of$179,035.75 to complete the exploratory testing at the Disston Avenue
potable supply well. Thus the net change order requested is $111,597.75. The new
proposed total contract value will be$644,972.75.
