PROPOSAL FOR PROFESSIONAL
SURVEYING AND ENGINEERING SERVICES
CITY OF CLERMONT
RECLAIMED WATER SYSTEM SUPPLEMENTAL SUPPLY WELL
I. PROJECT OVERVIEW
The City of Clermont (City) intends to utilize the existing well located on Hancock Road in front of
Windy Hill Middle School as a supplemental source of supply for the City's reclaimed water system.
Modifications to the existing well will include installation of a well pump, discharge piping, and
associated electrical/control with ultimate discharge of supplemental water into the proposed 3.0 MG
ground storage tank to be constructed at the City's Water Reclamation Facility (WRF). The well
head mechanical equipment will consist of a submersible well pump housed below grade in a precast
concrete vault with access hatch. This City will be responsible for design, permitting and
construction of the discharge piping from the well head vault to the WRF. The well is currently
permitted by the St Johns River Water Management District (SJRWMD) at an average day
withdrawal capacity 2.01 MGD with a total withdrawal capacity of 733 MG from 2011 to 2031.
Design of the well pump will be such to maximize use of the supplemental resource.
This proposal is for professional engineering services associated with survey, design, permitting,
bidding and award, and construction administration services for the Reclaimed Water System
Supplemental Supply Well project. The services provided by Tetra Tech consist of the following
phases and preferred in accordance with the City of Clermont/Tetra Tech professional engineering
services for public works projects agreement dated October 13,2009:

B. Survey
C. Permitting
D. Bidding and Award
E. Construction Administration
II. SCOPE OF WORK
A. Design/Preparation of Construction Documents
Design will result in preparation of the bidding and contract documents, engineering
drawings and technical specifications, which will be submitted to the City for review at
90- and 100-percent completion levels. The 90-percent submittal will include: plan,
section and required details of the well head and electrical/instrumentation drawings and
project technical specifications. The 100-percent submittals will include: updated
engineering drawings and technical specifications incorporating the comments received
from the City on previous submittals, and an opinion of project cost.

Three (3) sets of drawings and specifications will be provided to the City for each review.
A review meeting will be held with the City following the 90-percent submissions. To
ensure proper design of the facilities, Tetra Tech will obtain approval from the City for
any substantial changes in the preliminary design prior to incorporation in the final
design. Tasks to be completed during this phase are summarized below:
1. Prepare a design memorandum summarizing the basis of design and hydraulic
calculations.
2. Prepare construction drawings and technical specifications for the facilities
described herein previously. Documents shall include drawings and technical
specifications covering:
a. Cover sheet, index,key map, general notes, and project location map.
b. Detailed civil, mechanical, and electrical sheets. Drawings will be
prepared using AutoCAD Version 2012.

c. Prepare a project manual to include applicable technical specifications
(Divisions 2-16) for competitive bidding. The project manual and its
contents will be formatted in accordance with the Construction
Specification Institute (CSI) and will be prepared using Microsoft® Word.
The City will be responsible for preparation of standard bidding
requirements and general requirements for the project. Tetra Tech will
coordinate the project bid form and other requirements with the City's
Purchasing Department, as necessary.
d. Prepare a final opinion of construction cost based on previous bid
tabulations, vender quotes and estimates provided by contractors.
e. Provide the City two(2) complete sets of the final contract documents.

B. Survey
Tetra Tech will provide professional surveying services to:
1. During design, Tetra Tech shall provide a topographic survey of the project site (area
adjacent to supplemental well (100 ft x 100 ft). Establish horizontal and vertical
control based on Lake County datum NAD83 (2007) Florida East Zone, NAVD
1988.
2. Tetra Tech will provide a sketch of description for the proposed well site easement on
the Lake County School Board Property of Windy Hill Middle School,if required.
C. Permitting
Tetra Tech will prepare and submit the following permit application and supporting
documentation to the Lake County Public Works as required to secure permit for
construction and operation of the reclaimed water system supplemental supply well. Tetra
Tech will also respond to requests for additional information (RAIs) from permitting
agencies to clarify the original applications. All permit application fees will be paid by the
City.
1. Lake County Utility/Right-of-Way Utilization Permit Application. Tetra Tech will
arrange and attend a pre-application meeting with Lake County Public Works.
No additional permitting is anticipated to be required of the Florida Department of
Environmental Protection (FDEP) and St Johns River Water Management District
(SJRWMD). The supplemental supply well project was included as part of the permit
renewals with each agency: FDEP Domestic Wastewater Facility Permit (No. FLA010515-
010) and SJRWMD Consumptive Use Permit(No. 2478).
D. Bidding and Award
Upon authorization to proceed with the bidding and award phase of the project, Tetra Tech
will complete the following tasks:
1. Provide one (1) copy of the Contract Documents (engineering drawings and
specifications) in PDF format for distribution to potential bidders by the City's
Purchasing Department.
2. Prepare for and administer one (1)pre-bid conference.
3. Respond to questions and prepare addenda as required to interpret, clarify or
expand the bidding documents. The City Purchasing Department will distribute
all addenda.

4. Review and evaluate the apparent low bidder's qualifications for undertaking the
work and make recommendation of the award of the contract.
5. Prepare two (2) copies of a conformed set of Contract Documents for the City and
one (1) reproducible set for the recommended Contractor.
E. Construction Administration
Upon award of the project, Tetra Tech will assist the City with construction
administration and shall complete the following tasks based on a construction duration of
6 months:
1. Prepare for and administer one (1)preconstruction meeting.
2. Attend periodic progress and specially scheduled meetings throughout progress of
work. Progress meetings will be held as needed to coordinate work in progress.
It is anticipated that 4 meetings will be needed at an average duration of three (3)
hours each for preparation and attendance, followed by a site visit at an average
duration of one (1) hour each, based on an anticipated construction duration of 6
months.
4. Review shop drawings to determine conformance with the design concepts of the
project and compliance with the requirements provided in the contract documents.
5. Review requests for information (RFIs), provide interpretation of construction
documents,and issue written clarifications or interpretations.
6. Review the Contractor's Application for Payment and the accompanying data and
schedules, determine the amounts owed to the Contractor, and advise the City of
the recommended payments to the Contractor.
7. Upon written request by the Contractor, conduct a substantial completion
inspection of the project with the City and distribute a punch list of observed
deficiencies to be completed by the Contractor prior to the final completion date.
The project will be certified substantially complete only if the work is sufficiently
complete in accordance with the contract documents, so that the work can be
utilized for the purposes for which it is intended. Final completion inspection to
verify that the punch list items have been addressed will be performed by the
City.
8. Review the Contractor's as-built submittals monthly for adequacy and review
listing of deviations from the construction permit and approved construction
documents. Prepare record drawings for City's use from information provided by
the Contractor delineating the dimensions, location, and elevation of all facilities

constructed. Provide the City with one (1) CD-ROM electronic file of record
drawings in PDF format and two (2) sets of prints of the record drawings.
9. Prepare and submit certifications and required supporting documentation to
regulatory agencies having jurisdiction including Lake County Public Works.
III. COMPENSATION SUMMARY
The proposed compensation for the Scope of Services described in Section II is summarized below.
Attachment B presents a detailed breakdown of the estimated compensation for the Scope of
Services.
