Dear Mr. Kinzler:
This letter outlines a scope of services for tasks related to compliance with the Florida
Department of Environmental Protection (FDEP) Ground Water Requirements,of Domestic
Wastewater'Facility Permit No. FLA010515-010, dated December 28, 2012. Tasks proposed
herein include:
• review of site conditions and nearby monitor wells for preliminary design,
• discussion and confirmation of preliminary well design by the FDEP Central District,
• preparation of documents to procure bids from three licensed well drilling contractors,
• review of bids and recommendation of award,
• installation of one (1) background and one (1) compliance monitoring well,
• sampling of the wells for the analytes required by the FDEP,
• surveying the top-of-casing elevation and location of each new monitoring well, and
• completion of documentation well construction and sampling results.
In preparing this proposal, we reviewed the terms of the Domestic Wastewater Facility Permit
and the approved locations of the background and compliance wells provided by the City, and
relied on other relevant information concerning the Eastside reclaimed water system and local
topographic and geologic data.
We understand that time is of the essence and will provide you with a project schedule,
including estimates on the dates of drilling, sampling, and receipt of laboratory results as
quickly as possible after receipt of approval to begin work. We estimate that monitoring well
installation, development, testing, and sampling will require approximately two to three weeks

following approval of the proposed scope of work. Laboratory analyses of groundwater
samples from the new wells will require approximately one to two weeks. We will submit a
well installation and sampling report to the City within two weeks of receipt of laboratory
water quality results.
The proposed scope of services for this project is outlined below, and we propose to provide a
recommended drilling contractor within one week from NTP, and to complete "all" tasks
within 45 days from the notice to proceed. The proposed compensation for the Scope of
Services described herein will be billed on a time and material basis with an initial upset limit
of$4,903,based on the attached compensation summary.
SCOPE OF SERVICES
Task 1 —Preliminary Well Design/Bidding Services
Tetra Tech will complete tasks necessary to prepare documents to solicit bids from three (3)
licensed well,drilling contractors. Specific tasks include review of topographic maps, soils,
and water levels at the proposed site and review of monitor well designs from nearby sites for
comparison. We will prepare preliminary monitoring well designs suitable for the new
proposed well locations, such that line item quotes can be provided by Florida licensed well
drilling contractors. Following receipt of the three bids for the proposed monitor well
installation, we will review each bid and make a recommendation of award to the City.
Task 2—Monitoring Well Installation, and Sampling
Task 2 includes field oversight of installation of new monitoring wells and sampling of the two
new monitoring wells. Tetra Tech assumes the City will contract directly with the
recommended drilling contractor. The the,driller will collect lithologic samples using either
standard penetration test (SPT) or direct-push technology (DPT) borings. From these samples,
a Tetra Tech professional geologist will review lithologic samples at each well site to
determine the most suitable well designs. Each monitoring well will be installed in accordance
with FDEP requirements and will be documented on FDEP well completion report forms. The
driller will provide a well completion report on Water Management District forms. The driller
will engage the services of a Florida licensed surveyor to survey the location, top-of-casing
elevation, and land surface elevation. Those data will be included on the FEP well completion
report forms. Each new well will be developed until free from sand and silt and until turbidity
is less than 20 NTU. Following a wait of at least 48 hours, each well will be purged and
sampled for the analytes required by Section III.B.5 of FDEP DWF permit No. FLA010515-
010.

Task 3 —MW Installation Documentation
The results of well installation, testing, and groundwater sampling, will be documented on
FDEP monitoring well installation report forms and Water Management District well
completion report forms that are required for submittal by the FDEP No. FLA010515-010.
The well completion report will also include the results of groundwater sampling obtained
from the City's laboratory contractor.

The estimated cost and man-hours are provided in the attached price summary table for your
review. Our quote assumes that lab fees will be directly invoiced to the City and that the City
will engage the driller and surveyor. We appreciate the opportunity to continue to provide our
