Dear Ms. Dykehouse:
The Lake Hiawatha Preserve (formally the Inland Grove Property) was purchased by the
City of Clermont in 2006. Enhancements for the development will include restrooms
picnic pavilions, biking and hiking trails, a fishing pier and floating dock, a playground,
volleyball courts, and Roundabout. We understand that Phase 1 has been split into 2
Phases 1A and 1B. Phase 1A has bid and construction will begin shortly. The
Roundabout will be constructed concurrent with the Phase 1A project. The attached Scope
of Servics outlines the proposed services.
Attached for review is the proposals requested for professional services to assist the City
with completion of_project.

SCOPE OF SERVICES
CITY OF CLERMONT
LAKE HIAWATHA PRESERVE ROUNDABOUT
(CONSTRUCTION ADMINISTRATION SERVICES)
A. PROJECT DESCRIPTION
The City of Clermont(City)has completed design,permitting, and bidding of the Lake Hiawatha
Preserve Roundabout project. Tetra Tech will assist the City with administration services during
project construction. Tasks to be completed for this project are further described herein and will
be performed in accordance with the City/Tetra Tech Continuing Contract and Agreement for
Professional Engineering Services for Public Works Projects dated October 13, 2009.
B. SCOPE OF SERVICES
Upon award of the project, Tetra Tech will assist the City with construction administration and
shall complete the following tasks based on estimated construction duration of 120 days.
1. Review the contract documents and permits to understand the contract/permit
requirements of the project required to provide construction administration services and
regulatory certifications.
2. Attend periodic progress and specially scheduled meetings throughout progress of work.
Progress meetings will be held monthly and additional meetings as needed to coordinate
work in progress. It is anticipated that four (4) meetings will be needed at an average
duration of two (2) hours each, based on an anticipated construction duration of 120
calendar days. The Contractor will be responsible for recording and distributing meeting
minutes.
3. Make visits to the site at intervals appropriate to the various stages of construction in
order to observe the progress and quality of the Work. It is anticipated that four (4) site
visits with an average visit duration of approximately three (3) hours will be required,
based on an anticipated construction duration of 4 months and the various schedules of
the participating disciplines. Representatives from the civil and electrical disciplines will
visit the project site as outlined below:
• Civil—Site Work and Landscaping: 4 Site Visits
• Electrical—Site: 2 Site Visits
4. Review shop drawings up to two (2) times per submittal and required testing results to
determine compliance with the requirements provided in the contract documents.

5. Review requests for information (RFIs), provide interpretation of the construction
documents,and issue written clarifications or interpretations.
6. Review the Contractor's application for payment and the accompanying data and
schedules, determine the amounts owed to the Contractor, and advise City of the
recommended payments to the Contractor.
7. Upon written request by the Contractor, conduct a substantial completion inspection of
the Project with the City and distribute a punch list of observed deficiencies to be
completed by the Contractor prior to the final completion date. The project will be
certified substantially complete only if the work is sufficiently complete in accordance
with the contract documents, so that the work can be utilized for the purposes for which it
is.intended. Upon written request by Contractor, conduct a reinspection to confirm that
substantial completion punch list items have been addressed and subsequently provide a
substantial completion certification to the City.
8. Review the Contractor's as-built submittals monthly for adequacy and review listing of
deviations from the construction permit and approved construction documents as
prepared by the City's onsite inspection staff. Prepare record drawings for the City's use
from information provided by the City inspection staff and the Contractor delineating the
dimensions, location, and elevation of all facilities constructed (i.e. red lined drawings
and as-built survey). Provide the City with one (1) CD-ROM electronic file of record
drawings in AutoCAD current release, one (1) set of reproducible and two (2) sets of
prints of the record drawings.
9. Prepare and submit certifications and required supporting documentation to regulatory
agencies having jurisdiction (Lake County—Right of Ways Utilization Permit is the only
permit anticipated; SJRWMD — Environmental Resource Permit and FDEP — Water &
Wastewater Extension Permits are not anticipated).
C. COMPENSATION SUMMARY
The total lump sum compensation for the Scope of Services described in Section is $13,200.
-D. SCHEDULE
A general project schedule based on the construction contract is 120 calendar days from the
Contractors Notice to Proceed.
E. ASSUMPTIONS
The following assumptions were made in preparation of this proposal.
1. The City will provide conformed contract documents for use during construction in
electronic format(Drawings AutoCAD and Specifications MS WORD).

2. Copies of all permits obtained for construction of the project.
3. Copy of the site geotechnical and ecological reports.
4. The City will provide daily construction inspection.
5. The City will perform the final completion inspection of the Project to determine if the
substantial completion punch list has been addressed.
6. The Contractor will provide the contractor as-built drawings via hard copy and AutoCAD
format.
