Pursuant to your request,Andreyev Engineering, Inc (AEI)has completed this proposal to conduct a Phase II Environmental Site Assessment(ESA)for the subject property The scope of services for this proposal is designed to determine the presence of petroleum impacted soil in an area of proposed construction approximately 25 feet from an area that has been identified for investigation of petroleum constituents by the Florida Department of Environmental Protection (FDEP) The The following items are provided 1. Underground Storage Tank Information: The subject site is registered with the Florida Department of Environmental Protection (FDEP) as utilizing a 2,000-gallon diesel underground storage tank (UST) and a 2,000-gallon gasoline UST from 1971 to June 1992, when they were removed 2. Tank Closure Assessment Information: The USTs were removed from the subject site in June 1992, and disposed of by BETX Environmental Consultants Lake County personnel were present on-site during the removal of the USTs The tank closure documentation submitted to the Lake County Department of Planning and Services in June 1992 indicates that the two USTs and dispensers were removed As part of tank closure assessment activities, eleven soil borings were installed and soil samples were collected for analysis with an Organic Vapor Analyzer (OVA) The exact depths of the samples collected were not identified,and the OVA results were not provided in the tank closure assessment report for all of the samples The only OVA reading was for a soil sample collected from beneath the gasoline UST at approximately 5 feet, which detected a reading of 750 parts per million (ppm) It was reported that"one perforation approximately 14" diameter was located on the west end of Tank No 1 at the bottom seam area There was also some stained soil that was noticed under the dispenser locations A separate groundwater sample was obtained from the tank pit,and was analyzed for BTEX parameters, and was submitted to Lake County by the City of Clermont in March 1993 No levels of BTEX parameters were detected above 1 microgram per liter(ug/L) 3. Petroleum Discharge: A Discharge of Petroleum Products was reported as having occurred on June 5, 1992, as a result of the soil OVA level of 750 ppm The petroleum discharge on June 5, 1992, was determined by the Florida Department of Environmental Protection (FDEP)to be eligible for the Petroleum Cleanup Participation Program (PCPP)on May 23, 1997 4.
