PIGGYBACK CONTRACT
.
This contract is pursuant to our conversation today, regarding the City's West
Wastewater Treatment Plant, in regard to Piggybacking the City of Apopka,
Residuals, Hauling, Treatment and Disposal Contract #2001-01.
1. We hereby agree to piggyback this contract under all terms and conditions
for the term of the contract plus all annual extensions.
2. All employees of Shelley's Environmental Systems will be covered by
workman's compensation insurance,
3. All of Shelley's trucks and equipment will be properly insured licensed and
permitted,
4, Shelley's Environmental Systems will provide services as needed by the
City of Clermont, with twenty-four hour prior notice,
5. All residuals will be processed and treated at Shelley's Environmental
Systems Residuals Management Facility at Zellwood, Florida. (Florida
Department of Environmental Protection permit # FLA016177) ,
6, Shelley's will provide truck and pump trailers for hauling, with the City
providing a loader and operator for truck loading.
