TERMS AND CONDITIONS
MAINTENANCE AGREEMENT FOR CAL HEGSTROM OF FLORIDA
EQUIPMENT
In order to assist lis customers in maintaining their equipment in elliclent operating condllion, Cal Hegstrom of Florida (CHF) agrees to
furnish maintenance service and Install parts for the equipment 'described on the reverse side hereof at the rates there specllled and upon
the following terms and conditions.

This agreement shall be for an initial one-year period of service and Inspections ending on the date shown on the reverse side
hereof and therealter continues In ellect for similar one-year periOds unless cancelled.
Maintenance agreement charges are payable yearly In advance at the annual rate on the annual payment date shown on the
reverse side of this agreement.
ThiS maintenance agreement may be cancelled by either party as of the lirst day 01 the month following lilteen days writler notice.
In the event 01 cancellation, charQes will be prorated and CHF will refund the unearned portion of any amount prepaid hereunder.
CHF will provide a minimum 01 two Inspections yearly at appropriate intervals. Inspections may be made in conjunction with emer-
gency service calls. Emergency ser-vice calls will be made during regular business hours at no extra charge.
CHF will replace without charge parts which have broken or worn through normal use and are necessary for machine servicing
and maintenance adjustments, excepl accessories, supplies. and Drums. Also excluded Irom this agreement are modifications to the
equipment and overhauls.
6. This Agreement shall not apply to repairs made necessary by accident, misuse, abuse, neglect, thelt, vandalism. electrical power
failure, fire. water, or other casualty, or to repairs made necessary by service perlormed by personnel other than those 01 CHF.
7. The equipment shall be in good mechanical condition on the date of commencement of this Agreement.
B. This Agreement is not assignable and is automatically cancelled, with respect to the particular machine, should the equipment it
covers be sold to a third party.
9. The amount 01 this agreement shall be increased by an amount equal to any applicable tax now or herealter assessed. levied or
Imposed by any federal, state or local authority. '
10. Zones are defined in relation to the CHF service office providing service for the equipment covered hereunder. The applicable rates
for each Zone appear In CHF's price lists.
Zone I Within a ten (10) mile radius 01 the CHF service ollice or within Metropolitan
area.
Zone II Beyond Zone I, but withl~ a twenty (20) mite radius 01 the CHF office.
Zone III - . Beyond Zone II, but within a thirty-live (35) mile radius 01 the CHF ofllce.
Zone IV - Beyond Zone III, but within a sixty (60) mile radius of the CHF Service, office.
11. Where Indicated on the reverse side, the rates quoted In this agreement have .been adjusted to reffect extended machine use: "
the machine Is operated for two shills, one and one.half times the standard rate has been charged: where operated for three
shilts, two times the standard rate has been charged. When after regular-business'hours' service Is required on machines covered
by this contract, and If such service is available, regular after hours service charges will be made.
12. Service hours - Monday-Friday 8:30-5 PM. f.1lter 5 PM and Sat~rdays - Single call rate plus 50%. Sundays and Holidays -
Double the single rate. .
13. This service contract is Issued at a special price In consideration of the customer purchasing toners from CHF. If toners are
furnished by another supplier we will continue to service Ihe machine but only af our single call rate, plus parts.
