Memorandum of Understanding
By and Between
LifeStream Behavioral Center, Inc.
a n d
Clermont Police Department
THIS AGREEMENT, is originally made and entered into by and between Clermont Police Department
and LifeStream Behavioral Center, Inc. (hereinafter referred to as "LSBC.").
WHEREAS, LSBC desires to provide a Mobile Crisis Response Service in collaboration with
the Clermont Police Department;
WHEREAS, Clermont Police Department seeks a behavioral health stakeholder in support of
its efforts to assist individuals experiencing a mental health or substance use crisis;
WHEREAS, LSBC agrees to provide a professional staffmember to offer MH/SUD crisis
intervention, evaluation, and counseling; case consultation, crisis case management and other
services as agreed on;
WHEREAS, the parties desire to reduce their agreement to writing.
NOW, THEREFORE, in consideration of the premises, and the mutual promises contained
herein, and other good and valuable consideration, the receipt and adequacy of which is hereby
acknowledged, Clermont Police Department and LSBC agree as follows:
I. LSBC will perform SUD/MH crisis intervention, evaluation, and counseling; case
consultation, crisis case management and other services as agreed on with adults and
juveniles referred by Clermont Police Department.
2. LSBC will provide these services at no direct war to the Clermont Police Department;
3. LSBC will access funding available through Lake County under the Justice and Mental
Health Collaboration Program- Planning and Implementation Grant, for which LSBC is
subcontracted to deliver MCR services.
4. 545,000 of MCR funds received from Lake County under the BJA grant # 20IS-1390
will be reimbursed to the City of Clermont to partially support the salary of an officer
dedicated to the MCR program as a co -responder for mental health and substance use
related crisis calls
5. LSBC will respond to requests for consultation with Clermont Police Department
regarding individuals in the community with behavioral health challenges leading to
frequent contact with law enforcement officers.
6. LSBC will provide program consultation as requested.
7. LSBC will provide Crisis Intervention Team Training opportunities for Clermont Police
Department personnel.
Clermont Police Department shall be responsible for the following:
1. Working collaboratively with the Mobile Crisis Response staff member on duty, contacting
the MCR staff member for assistance with calls involving adults and juveniles with a mental
health or substance use disorder crisis or concern.
2. Making referrals for assessment and services with provision of any relevant background and
contact information;
3. Providing workspace within the Clermont P.D. for the Mobile Crisis Response on -duty staff
during normal business hours.
4. Assigning an officer to partner as a co -responder with the MCR staffmember on duty.
5. Assisting in the planning and delivery of Crisis Intervention Team Training, along with
designated LifeStream staff members, to Lake County law enforcement organizations
choosing to participate.
6. Sending officers to Crisis Intervention Team (CIT) Training.
5. Providing support m encourage the Departmenfs full participation in the program, sharing
information and actively engaging in goal accomplishment.
6. Providing information and training about Clermont Police Department to LSBC staff for
purposes of mutual understanding and collaboration.
7. Requesting completion of releases of information between LSBC and Clermont Police
Department for citizens receiving Mobile Crisis Response Services.
LSBC shall be responsible for the following:
A. Services described above.
B. Consultation and collaborative staffing availability
C. Medical Records Management for behavioral services.
D. Billing the County for behavioral services under the grant.
The term of this agreement shall be for one year beginning upon the date this agreement is
signed by both parties. This agreement may be mewed for up to two consecutive one year terms
upon mutual written consent of both parties. This agreement may be terminated with or without
cause by either party upon giving thirty (30) days written notice sent as specified in NOTICE
section of this agreement.
All notices, requests, demands and other communications with respect hereto will
be in writing and will be delivered either by hand, with proof of delivery, or sent prepaid by air

courier or sent by the United States mail, certified, postage prepaid, return receipt requested, at the
addresses designated below:

This agreement is made and entered into in the State of Florida, and shall be construed, performed,
and enforced in all respects in accordance with the laws, rules, and regulations of the State of Florida -
Nothing herein is intended to serve as a waiver of sovereign immunity by any party to which
sovereign immunity applies. Nothing herein shall be construed as consent by a State Agency or
subdivision of the State of Florida to be sued by third parties in any matter arising out of any contract
or agreement
By signing below, each signatory represents that they have the authority to sign on behalf of their
respective organizations.
