The Department of Health and Rehabilitative Services hereby contracts
with to rovide

community service work program site(s) for children who have, pursuant to
la~•r, been made the responsibility of the Department of I?ealth and Rehahili-
tative Services. On-site supervision of children involved in this work
program will. be ttie responsibility of the !+: ty ~ ~~,_,,,,,,,r T F~~~a
Tile Department having the primary responsib~.Llity will provide that workmen`s
compensation coverage required by Section 39.1I(6), Florida Statutes, and
further authorized by Section 440.10{1), Florida Statutes, and will at all
times have access to its juveniles who are performing in such work'prograrn
so as to verify the appropriateness of the prograrn.
This agreement is entered into by both parties on the 15th day
of October , 1981 and will be considered in effect until
one or both of the parties wishes to withdraw from this contract and duly
notifies the other party of that action. At that point, this contract is
considered null and void.
