to assure that your Copy Equipment is kept in efficient operating condition. For an annual fee, our factory-trained
personnel will perform the following services on the equipment listed below for one year in accordance with the terms
and conditions set forth on the reverse side of this page.

TERMS AND CONDITIONS
MAINTENANCE AGREEMENT FOR CAL HEGSTROM OF FLORIDA
EQUIPMENT
In order to assist its customers in maintaining their equipment in efficient operating condition, Cal Hegstrom of Florida (CHF) agrees to
furnish maintenance service and install parts for the equipment described on the reverse side hereof at the rates there specified and upon
the following terms and conditions.
t. This agreement shalt be for an initial one-year period of service and inspections ending on the date shown on the reverse side
hereof and thereafter continues in effect for similar one-year periods unless cancelled.
2 Maintenance agreement charges are payable yearly rn advance at the annual rate on the annual payment date shown on the

3. This maintenance agreement may be cancelled by ether party as of the first day of the month following fifteen days written notice.
In the event of cancellation, charges will be prorated and CHF writ refund the unearned portion of any amount prepaid hereunder.
4 CHF will provide a mrmmum of two inspections yearly at appropriate intervals. Inspections may be made m conjunction with emer-
gency service calls. Emergency service calls •,vill be made during regular business hours at no extra charge
5 CHF will replace without charge parts which have broken or worn through normal use and are necessary for machine servrcrng
and maintenance adjustments, except accessories. supplies. and Drums. Also excluded from this agreement are modifications to the
equipment and overhauls.
6. This Agreement shalt not apply to repairs made necessary by accident, misuse, abuse, neglect. theft, vandalism, electrical power
failure, tire. water, or other casualty, or to repairs made necessary by serwce performed by personr.~. other than those of CHF.
7 The equipment shalt be in good mechanical condition on the date of commencement of this Agreement
8. This Agreement is not assignable and is automatically cancelled, with respect to the particular machine. should the equipment ~t
covers be sold to a third party.
9. The amount of this agreemem shall be increased by an amount equal to any applicable tax now or hereafter assessed. leveed or
imposed by any federal, state or local authority
10. Zones are defined in relation to the CHF service office providing servrce for the equipment covered hereunder. The applicable rates
for each Zone appear in CHF's pace lists.
Zone I - Within a ten (10) mile radius of the CHF service office or wdhin Metropolitan
area.
Zone II - Beyond Zone I, but within a twenty (20) mile radius of the CHF office.
Zone III - Beyond Zone II, but within a thirty~hve (35) mile radius of the CHF office.
Zone IV - Beyond Zone IIt. but within a sixty (60) mile radws of the CHF Service office.
11. Where indicated on the reverse side, the rates quoted m this agreement have been adjusted to reflect extended machine use'. If
the machine is operated for two shifts. one and one-halt times the standard rate has been charged; where operated for three
shills. two times the standard rate has been charged. When after regular-busmess•hours' serwce is required on machines covered
by this contract, and if such service ~s available. regular alter hours service charges will be made.
t2. Service hours -Monday-Friday 8'.30-5 PPA After 5 PM and Saturdays -Single calf rate plus 50%. Sundays and Holidays -
DOUble the Single rate.
13. This service contract is issued at a special price m consideration of the customer purchasing toners tram CHF. I} toners are
furnished by another supplier we will continue to servrce the machine but only at our single call rate. plus parts.
