The services specified bjelow are required in order to complete the above-referenced
project. We have outlin~d each individual task that is required. Your signature below
will constitute approval df said Change Order Request which will in turn be incorporated
into your existing contract.
TASK 004 ROUTE SVJRVEY
Conduct Route Survey with topography to include right of way lines, trees,
pavement, curbing, visible utilities or underground utility markings, and any other
above ground vis~ble features within the corridor. Route survey to include
Bowman Street frf om Almond Street 700 LF south, Almond Street from Bowman
Street east 500' tb Anderson Street, Anderson Street 400 LF south from Almond
Street and appro~Cimately 400 LF between homes from intersection of Bowman
Street/Sunnysidel Drive to Sunnyside Drive including lake frontage on Sunnyside
Drive.
FEE: $8,800.00
TASK 005 FINAL ENGINEERING PLANS ON DRAINAGE COLLECTION SYSTEM
Prepare enginee ing plans for the construction of a drainage
collection/transm ssion system along Bowman Street, Almond Street, and
Anderson Street s described in Task 004 Route Survey above. This task will
include calculati gsub-basins for inlet locations, spread calculations, stormcad
model for pipe si ing and end treatment design for erosion control at Lake
Sunnyside. B.E. .H. will also design a stormwater treatment baffle box for
removal of heav sediments, trash, oils and grease to be installed between
Sunnyside Drive and Lake Sunnyside.
Submittal will be ade to the City of Clermont at 80% and 100% completion for
review. The City of Clermont will coordinate with property owners to negotiate
stormwater drain~ge easement and access to private property for survey.
B.E.S.H. will pre are sketch and legals for the City to obtain easements to be
billed under origilhal contract Task 003.
