PROJECT DESCRIPTION In order to improve finished water quality and system reliability in the western potable water service area, the City of Clermont (City) intends to construct a new water treatment plant (WTP) that will centralize water treatment and high service pumping and provide additional system storage. The proposed Westside WTP is anticipated to be constructed on a 3.3 acre parcel bounded on the west by Disston Avenue, the east by the Winn -Dixie Shopping Center, the south by Dairy Queen/Ay Jalisco restaurants, and the north by E. Highland Avenue as illustrated in Figure 1. In general, the new facilities will include a raw water supply well, ground storage tank, chemical feed facilities, high service pumps, standby power, electrical/instrumentation systems, and associated raw and finished water transmission mains. Specific details regarding the contemplated facilities and services are presented below: 1. The Westside WTP will have a design capacity of 4.4 million gallons per day (MGD) on a maximum day demand basis per the City's 2008 Potable Water Utility Master Plan. Water treatment may consist of conventional tray aeration or degasification with carbon dioxide chemical feed facilities both for sulfide reduction as required based on the raw water quality and finish water quality goals, and disinfection with sodium hypochlorite. Raw water supply will include interconnecting the existing Grand Highway well, the existing Fourth Street well, and the newly constructed Disston Avenue well to the proposed WTP. 2. Major facilities include a 1.00 to 1.50 million gallon (MG) prestressed composite ground storage tank with cascade tray aerator or degasification with odor control, high service pumps and electrical equipment located inside a concrete masonry building (approx 40 ft x 60 ft) with sound attenuation, and sodium hypochlorite chemical storage and feed system located inside a concrete masonry building (approx. 30 ft x 40 ft). This proposal was based upon a solution feed type carbon dioxide chemical storage and feed system, if required. 3. A standby power generator located on an exterior slab at grade within an aluminum sound attenuating enclosure. The emergency generator will be capable of powering the WTP to include but not limited to; the on -site well, the high service pumps, chemical feed systems, controls, and site lighting. A new 24 -inch raw water supply well is proposed to be constructed on the WTP site as part of a separate contract. The existing Grand Highway and Fourth Street wells will be modified to include a new well pump, electrical /instrumentation systems, standby power generator and masonry building modifications. It is anticipated that the standby power generator at each of the existing well sites will be located on an exterior slab on grade with an aluminum sound attenuating enclosure. The proposed well pump and local appurtenances will be suitable for outdoor use constructed slab on grade, no enclosure is proposed. 5. An instrumentation system will be provided for local/remote control and monitoring of the WTP and the two remote well sites. The system will be based on using Allen - Bradley programmable logic controllers (PLCs) and will include a SCADA system that is compatible with other facilities owned by City. 6, A security system will be provided to facilitate remote monitoring of the WTP and the two (2) remote well sites. The system will be based on using a system that is compatible with other facilities owned by the City, 7. Miscellaneous site improvements of the WTP will include landscaping, irrigation system, site perimeter precast concrete wall, and coordination for relocation of wireless communication equipment presently onsite. 8. Approximately 11,400 linear feet (LF) of 16 -inch diameter raw water main to interconnect the two (2) existing remote wells to the proposed WTP site as illustrated in Figure 1. General alignments of the raw water mains are as summarized below: Grand Highway Well (Approx. 5,750 LF) • South on Bloxam Avenue to E. Desoto Street • West on E. Desoto Street to Disston Avenue • South on Disston Avenue to the Westside WTP Fourth Street Well (Approx. 5,650 LF) • East on W. Magnolia Street to 3rd Street • South on 3 Street to W. Minnehaha Avenue • East of W. Minnehaha Avenue to Disston Avenue • North on Disston Avenue to the Westside WTP 9. Approximately 2,650 LF of 24 -inch finished water transmission main to interconnect the proposed WTP discharge piping to the existing distribution system at the intersections of E. Highland Avenue and Drew Avenue and E. Highland Avenue and Bloxani Avenue as illustrated in Figure 1. SCOPE OF SERVICES A. Preliminary Design The purpose of the preliminary design phase is to gather information, develop and present the basis of design, and to establish the configuration of the proposed facilities. Tasks to be completed in this phase are described below: 1. Attend a project kick -off meeting with the project team and City staff to establish lines of communication, contract requirements, and schedule requirements. Tetra Tech will be responsible for recording and preparing meeting minutes for distribution. 2. Collect and review available information on existing utilities, maps, surveys, easements, plats, property ownership, aerials and proposed current and master planned roadway improvements along the potential project corridors that have an impact on the planning, design, permitting and construction or operation of the proposed WTP, remote wells and offsite pipeline improvements. 3. Evaluate potential corridors based on information gathered, methods of construction and provide a route recommendation for the proposed raw water main from the two (2) remote well sites to the Westside WTP. 4. Establish the design basis for the raw water supply system, water treatment process and equipment and facility finished water storage sizing analysis. Perform design calculations, select equipment, and develop process and chemical feed schematics and process mechanical layouts of new facilities and piping systems. Preliminary equipment selections and manufacturer catalog information. 5. Review the City's existing water distribution system hydraulic model to determine the WTP discharge pressure requirement of the finished water distribution system. 6. Establish the design basis for the stormwater management system and provide preliminary sizing for stormwater system required to support the project. Prepare an overall site plan to include the proposed new structures, driveways, stormwater management system and future expansions illustrating their relationship to the site boundary. 7. Regulatory design requirements. Prepare for and attend a pre - application meeting with the Florida Department of Transportation (FDOT) to discuss the permitting requirements of the project. 8. Prepare a design report that presents the results of the various evaluations, the basis of design for the recommended facilities, and the size and configuration of the facilities with their related opinion of cost. Attend a review meeting with the City staff to discuss the design report, modify the report as required, and develop concurrence regarding the proposed facilities prior to final design. Provide the City three (3) copies of the "final' preliminary design report. B. Final Design Upon acceptance of the preliminary design report by the City, Tetra Tech will initiate the final design of the Project. The final design will result in preparation of the bidding and contract documents, engineering drawings and technical specifications, which will be submitted to the City for review at 50 -, 90- and 100 - percent completion levels. The 50- percent submittal will generally include: water treatment plant civil and process mechanical drawings for the major facilities that incorporate the findings, recommendations, and approved layouts contained in the preliminary design report, initial structural, architectural and electrical /instrumentation drawings, raw /finished water mains — plan view, and draft project specifications (technical). The 90- percent submittal will generally include: updated engineering drawings (90% complete for all disciplines and profile for off -site pipelines), specifications (technical and bidding) incorporating the comments received from the City on the 50 percent submittal, and an updated opinion of cost. Three (3) sets of drawings and specifications will be provided to the City for each review. A review meeting will be held with City following each submission. To ensure proper design of the facilities, Tetra Tech will obtain approval from the City for any substantial changes in the preliminary design prior to incorporation in the final design. Tasks to be completed during this phase are summarized below: 1. Prepare a comprehensive project manual that contains bidding and contractual documents (Divisions 0 and 1) and technical specifications for competitive bidding (Divisions 2 through 16). The project manual and its contents will be formatted in accordance with the Construction Specification Institute (CSI). 2. Permitting Tetra Tech will prepare and submit the following permit applications and supporting documentation to the Florida Department of Environmental Protection (FDEP), Florida Department of Transportation (FDOT), Lake County /City of Clermont Public Works, St Johns River Water Management District (SJRWMD) and the City of Clermont Planning and Zoning Department as required to obtain permits for construction and operation the © TETRA TECH JDF /slm /sma/corresp/Revised Attach A -WTP Scope 12- 22,doc Tt # BP Clermont -6- 122810 proposed facilities. Tetra Tech will also respond to requests for additional information (RAIs) from permitting agencies to clarify the original applications. All permit application fees will be paid by the City. 1. Prepare and submit FDEP Permit Application for a Specific Permit to Construct PWS Components, Form 62- 555.900(7) (off -site pipelines, wells, and WTP). 2. Prepare and submit a "Notification/Application for Constructing a Domestic Wastewater Collection/Transmission System" to the FDEP. 3. Prepare and submit an application for a SJRWMD Environmental Resource Permit (ERP). It is anticipated that construction within wetlands and mitigation are not required. 4. Prepare and submit right -of -way utilization permit applications to FDOT and Lake County and/or the City of Clermont Public Works. 5. Prepare and submit an application for site development review to the City's Planning and Zoning (P &Z) Department. Attend one (1) meeting with the City's P &Z Staff. Respond to the P &Z's comments and resubmit final documents for approval. D. Bidding Upon advertisement of the project by the City, Tetra Tech will assist the City with bidding of the project and shall complete the following tasks: 1. Provide one (1) copy of the Contract Documents (engineering drawings and specifications) in PDF format for distribution to potential bidders by the City's Purchasing department. 2. Prepare for and administer one (1) pre -bid conference. 3. Respond to questions and prepare addenda as required to interpret, clarify or expand the bidding documents. The City Purchasing department will distribute all addenda to bidders. 4. Review and evaluate the apparent low bidder's qualifications for undertaking the work. 5. Consult with and advise the City as to the acceptability of subcontractors and other persons and organizations proposed by the prime contractor (hereinafter called "Contractor "). Consult with and advise the City as to the acceptability of substitute materials and equipment proposed by the Contractor when substitution prior to the award of contracts is allowed by the bidding documents. 7. Assist the City in evaluating bids and make recommendation of the award of the contract; and attend one (1) City Council meeting to answer any questions relating to the recommendation of award. 8. Prepare two (2) copies of a conformed set of Contract Documents for the City and one (1) reproducible set for the recommended Contractor. E. Construction Administration Upon award of the project, Tetra Tech will assist the City with construction administration and shall complete the following tasks based on a construction duration of 12 months: 1. Prepare for and administer one (1) preconstruction meeting and prepare written minutes of the meeting for distribution to attendees. 2. Attend periodic progress and specially scheduled meetings throughout progress of work. Progress meetings will be held monthly and additional meetings as needed to coordinate work in progress. It is anticipated that 14 meetings will be needed at an average duration of two (2) hours each, followed by a site visit at an average duration of one (1) hour each, based on an anticipated construction duration of 12- months. 3. Make visits to the site at intervals appropriate to the various stages of construction in order to observe the progress and quality of the Work. It is anticipated that 4 -site visits with an average visit duration of approximately three (3) hours will be required, based on an anticipated construction duration of 12- months. Representatives from the architectural and electrical disciplines will perform two (2) site visits at intervals appropriate to the various stages of construction. 4. Review shop drawings to determine conformance with the design concepts of the project and compliance with the requirements provided in the contract documents. 5. Review requests for information (RFIs), provide interpretation of construction documents, and issue written clarifications or interpretations. 6. Review the Contractor's Application for Payment and the accompanying data and schedules, determine the amounts owed to the Contractor, and advise the City of the recommended payments to the Contractor. After successful demonstration tests and upon written request by the Contractor, conduct a substantial completion inspection of the project with the City and distribute a punch list of observed deficiencies to be completed by the Contractor prior to the final completion date. The project will be certified substantially complete only if the work is sufficiently complete in accordance with the contract documents, so that the work can be utilized for the purposes for which it is intended. 8. Upon written request by the Contractor, conduct a final completion inspection of the Project with the City to determine if Work is finally complete and compile and distribute a punch list of items to be addressed. Upon written request by Contractor, conduct a re- inspection to confirm that final completion punch list items have been addressed and subsequently provide a final completion certification to the City. 9. Review the Contractor's as -built submittals monthly for adequacy and review listing of deviations from the construction permit and approved construction documents. Prepare record drawings for City's use from information provided by the Contractor delineating the dimensions, location, and elevation of all facilities constructed. Provide the City with one (1) CD -ROM electronic file of record drawings in PDF format and three (3) sets of prints of the record drawings. 10. Prepare and submit certifications and required supporting documentation to regulatory agencies having jurisdiction. F. Survey and Geotechnical 1. During preliminary design, Tetra Tech shall provide a boundary and topographic survey of the project site (remaining area not previously surveyed 3.3 acres total) and the two (2) remote well sites (Grand Highway well (0.01 acre) and Fourth Street wells (1.46 acres)). Establish horizontal and vertical control based on Lake County datum NAD83 (2007) Florida East Zone, NAVD 1988. Review deed of record for determination of existing property boundary and easements. The cost of supplemental title work, if required, will be paid by the City, 2. During final design, Tetra Tech shall provide a topographic /route survey for approximately 11,400 linear feet for the pipelines to interconnect the two (2) remote wells (Grand Highway and Fourth Street wells) to the Westside WTP and for the finished water main from the WTP west and east to the intersections of E. Highland Avenue and Drew Avenue and E. Highland Avenue and Bloxam Avenue, respectively. Survey shall include the following: a. Establish all horizontal and vertical control for a topographical survey of the project based on state plane coordinates NAD83 (2007) Florida East Zone, NAVD 1988. Survey data shall include topographic information, baseline control points every 1,000 feet (maximum spacing), vertical control points, section corners, subdivision control points, PRMs, R/W monuments, spot elevations, subdivision control points. The utility corridor to be surveyed includes half width of the right -of -way. c. Review right -of -way deeds of record and recorded plats for determination of existing right -of -way or easement widths along the route. Right -of -way or easement and property boundaries shall be plotted on survey drawings for the project route. The cost for supplemental title work, if required, will be paid by the City. d. Cross section elevations shall be shown to describe the nature of the topography and shall be provided at a minimum spacing of 50 feet along the proposed route to identify changes in grade. Cross sections shall also be provided at locations determined by the Engineer to clarify the nature of the proposed work. Water surface elevations of all water bodies shall be ascertained and the survey will provide both elevations and date. e. Improvements along the corridor will be surveyed and elevations taken. In general, the following entities shall be shown and identified: existing right -of -way, existing easements, swales, wetland boundaries as flagged by others, cross sections of topography, existing buried utilities, structures, tree trunk and foliage outline for trees 4" in diameter and larger, foliage outlines, sidewalks, culverts, end walls, water bodies, curbing, fences, driveways, valve boxes, utility poles, fire hydrants, utility boxes, utility markers, roadway (paved or unpaved), manholes, and soil boring locations. For the right -of -way surveyed, the existing underground water and sewer utilities will be field located by the City and existing underground power, communication and gas will be field located by Southeastern. f. Set horizontal control points throughout the project route. These points will consist of iron rebar with caps and shall be set at all baseline turns. g. Determine by excavation the horizontal and vertical location of the existing pipes at proposed "tie -in and critical" locations. Up to four (4) soft digs have been estimated for the project. 3. During final design, Tetra Tech shall arrange and pay for an independent soils investigation/testing firm to provide necessary soils data and other pertinent information required for design of the following, Soil borings logs and classifications, existing groundwater levels and estimated seasons high levels, structural foundation requirements, and pipe trench preparation and backfill requirements will be submitted in report format. ej TETRA TECH JDF /slm/sma/corresp/Revised Attach A -WTP Scope 12- 22.doc Tt # BP Clermont -10- 122810 a. Design of the proposed structural and stormwater management elements of the WTP. It is estimated that the investigation will include one (1) SPT borings to a depth of 100 feet, three (3) SPT borings to a depth of 50 feet, and three (3) SPT borings to a depth of 20 feet, two (2) auger borings to a depth of 20 feet and field permeability test. b. Design of the proposed off -site raw and finished water pipelines. It is estimated that the investigation will include 28 auger borings to a depth of 10 feet and four (4) SPT borings to a depth of 15 feet. F. Assumptions 1. Both the Westside WTP and off -site pipelines will be bid and constructed as one (1) project. 2. Any water quality laboratory analyses required of the project will be performed or paid for by the City. 3. No impacts to jurisdictional wetlands or threatened/endangered species are anticipated with development of the WTP site and off -site pipelines. 4. Design and construction of the relocated wireless communication equipment will be completed by the communication service provider. 5. All permit application fees will be paid by the City. 6. Supplemental title work for private properties, if required, will be paid by the City. III. COMPENSATION SUMMARY The total lump sum compensation for the Scope of Services described in Section II is summarized below.
