PROJECT DESCRIPTION office©chpengr.com The project consists of the addition of street lighting and related power systems for the following: 1. Lake Avenue. (2 blocks ) 2. West Desoto Street (3 blocks) Florida 3. West Avenue. (2 Blocks) 1051 Winderly PI. 4. Seventh Street. (One Block) 5. Redesign of surface parking located on Lake Avenue. This lighting is presently Suite 101 designed to be circuited from Jenkins Auditorium. It wi11 be redesigned to be served from the electrical service serving the street lighting. 6. Miscellaneous power receptacles located in medians and islands. Maitland, Florida 32751 BASIC SCOPE OF SERVICES a (407) 660 - 9099 Our basic scope of services includes the following: Construction Document Phase: fax: (407) 660 -8702 The Construction Document submission will include the following: 1 Final selections and specifications for all electrical systems included in the scope of work Timothy McNicholas October 27, 2011 Page 2 2. Specifications for all work included in Division 26 3. Permitting services with the City of Clermont. Construction Administration Phase: Construction Administration services will consist of the following 1 Job site visits for inspections or site meetings and shall be limited to two (2) which includes one final punch list. 2. Shop drawing/submittals review is included in our proposal. 3. Incidental engineering support (answering RFI's. answenng questions, and making clarifications) is included in our proposal. EXCLUSIONS, QUALIFICATIONS, and KEY ASSUMPTIONS 1. Cost estimating is not included in our proposal. 2. Structural Engineering is excluded from our proposal. 3. Civil Engineering is excluded from our proposal. 4. Should the Project be designed and constructed in multiple phases, our firm will review the fees for Construction Documents and Construction Administration services and will adjust those amounts accordingly. This proposal is based on the engineering and construction being performed under one phase /one Project. 5. Selection of street light fixtures shall be by others. 6. Engineering or redesign for unforeseen job site conditions is excluded. ADDITIONAL SERVICES Any major changes after commencement of the Construction Document phase or change in scope prior to that phase may be considered an additional service CHPA reserves our right to request additional services based on the extent of those and any other changes. Other examples of additional services are identified as exclusions or qualifications previously in this proposal. Compensation for those services shall be hourly or lump sum based on our standard billing rates Principal: $225 00 per hour ... .......................... It does not include reimburables• Construction Documents. $8,292 00 Construction Administration 2.764.00 Total Lump Sum Fee: $11,056.00 TERMS OF PAYMENT All engineering work for Basic Scope of Services and all approved additional services shall be invoiced.on a monthly basis. Payment shall be net 45 days after receipt of invoice REIMBURSABLES Reimbursables will be invoiced at cost (without markup), are not included in our firm's basic scope of services fee, and shall include the following: 1. Courier and overnight delivery. 2. Pnntmg and plotting for submittals identified herein 3. Overnight lodging and subsistence outside the Greater/Metro Orlando area.
