The services specified below are required in order to complete the above - referenced project. We
have outlined each individual task that is required. Your signature below will constitute approval
of said Change Order Request which will in turn be incorporated into your existing contract.
TASK 005 PHASE 3 UTILITY ADJUSTMENTS
Prepare Utility Adjustment Plans, Utility Work Schedule and associated F.D.O.T. utility
permits for utility adjustments associated with S.R. 50 and U.S. 27 Phases 2 and 3
construction plans. Task includes meetings with F.D.O.T. and City as requested, F.D.E.P.
permit and clearances if required.
Note: F.D.O.T. roadway project is a design /build project and has continuously changed since the
initial meetings with F.D.O.T. and Atkins. Based upon our last meeting with Atkins, Prince
Construction and F.D.O.T., this Phase 3 utility adjustments should be the final adjustments
relating to City of Clermont utilities.
FEE: $11,320.00 (TO BE BILLED HOURLY / NOT TO
EXCEED)
