BACKGROUND Increased treatment costs of the Westside Water Treatment Plant has led the City of Clermont to explore an alternative of eliminating "bad actor"wells in the western water distribution system and supplying water from the existing eastern water distribution system. Utility infrastructure is an important component to support residents and businesses, while maintaining reliability Interconnection of the east and west potable water distribution systems require rational common design approaches, quality assurance/quality control procedures and acceptance testing during installation. A hydraulic model evaluation was performed on the interconnect alternative by others. The evaluation demonstrated to have the potential ability to meet west-side demands while reliably meeting disinfection by-product limits and maintaining chlorine residuals within certain limits Therefore, this scope does not include hydraulic or water quality modeling. The City of Clermont desires to interconnect the east and west potable water distribution system as an alternative to constructing the West Side Water Treatment Plant. The proposed interconnects will be located at the following locations 1 Grand Highway—6-inch to 6-inch 2 Pitt Street—12-inch to 12-inch 3. Hook Street-12-inch to 12-inch • Each interconnection will be in a vault and include a pressure reducing valve, flow meter, chlorine analyzer and pressure recorder. Pitt Street and Hook Street will have remote actuated control valves to turn flow on and off, while Grand Highway will have the option Engineers•Surveyors•Architects(AA26000926)•Planners•Landscape Architects•Environmental Scientists•Construction Management•Design/Build 0 to include the controls. Controls will be integrated by others into the SCADA system to remotely monitor chlorine residual, pressure and flow and have the ability to remotely actuate the control valves. CLOSING The proposal submitted herein is intended to meet the criteria and is being submitted in accordance with the Executed Agreement dated the 13th day of October 2009, between the City of Clermont (CITY) and the firm of CPH, Inc , (ENGINEER) and extended on August 17, 2012. We are providing a detailed Scope of Services, as Attachment A, which presents a description of each task for the proposed Scope of Services for the Project. Also, the Attachment A shows the lump sum costs for each of the tasks, which is based on our understanding of the associated tasks. The total project lump sum fee as described herein is $42,550.00. Prepare for and conduct a project initiation/kick-off meeting to establish lines of communication, contract requirements, schedule and reporting requirements. 2. Collect and review project related available information such maps, aerials, surveys, easement descriptions, ROW records, plans, record drawings, soils investigation reports, privately owned utility system data, zoning classification, building codes and standards that may be pertinent to execution of the Project. Review requirements of agencies having jurisdiction over the Project. Collect and review other information that may have a bearing and impact on the planning, design, approval, permitting, construction or operation of the Project. 3. Evaluate existing conditions within the interconnection corridor by site visitation. Consider current field conditions and proposed site improvements and/or changes that may impact the Project and recommended location of the proposed interconnection 4. Prepare a preliminary layout (30% Plan Set) of the three (3) proposed interconnections and indicate major conflicts with existing utilities and areas where special construction techniques should be considered. Additionally, present other pertinent information necessary for the City to evaluate the proposed interconnection sites. 5. Prepare a preliminary estimate of probable construction costs for the interconnect improvements based on the preliminary layout. 6. Prepare a "Design Memorandum" which presents the findings and conclusions that resulted from the Preliminary Engineering effort, at a minimum, include illustrative sketches/plans (pipe alignment lay-out), permit requirements, future plans of other utilities and agencies that may affect Project (if applicable) and the conclusions and recommendations concerning further execution of the Project. 7. Submit three (3) Draft copies of the Design Memorandum for the interconnect improvements to the City, including a QA/QC Certification. Meet with the City to discuss the Draft Design Memorandum; collect review comments and prepare a comment tracking sheet. 8. Revise the Memorandum in accordance with the comments received from the City. Submit three (3) Final copies of the Design Memorandum to the City TASK 2—Survey and Geotechnlcal Investigations 1. Provide a Survey of the three (3) proposed interconnect locations. A-1 East-West Interconnect Scope(23-Sept-2013)dacx a. Specific Purpose Survey Existing plats and land records containing the project rights-of-way shall be obtained and reviewed. No title work fees are included within this proposal but provided documents/easements will be reviewed. Sufficient monumentation will be recovered, field located and verified in order to calculate and determine the right-of-way lines through the project area, as well as any platted easements adjacent to the right-of-ways. b. Topographic Survey The horizontal and vertical spatial relationship of the above ground natural or man-made features lying within the limits of survey defined will be established and mapped. Elevations shall be taken within each of the three 100 ft x 50 ft areas. Spot elevations shall be taken as necessary to identify significant elevation changes occurring within the limits of survey. Isolated trees having a diameter of six (4) inches(measured three feet above the ground level) lying within the limits of survey, shall be located and dense tree lines will be mapped. c. Horizontal and Vertical Control The project coordinate system will be based horizontally on the North American Datum 83 NAD 83(2007). The project will be referenced to state plane coordinates by field locating published control points. The National Geodetic Survey and Lake County control points will be researched and verified in the field This base of reference will be used to establish the horizontal coordinate system for the project. The project will be vertically based on the North American Vertical Datum 88 (NAVD 88). The project will be referenced to these published elevations by field locating published benchmarks. The National Geodetic Survey and Lake County benchmarks will be researched and verified in the field Horizontal and vertical control points both will be established for the project as required by the Minimum Technical Standards of the State of Florida d Subsurface Utility Designation The following service will be provided by a qualified sub-consultant (Southeastern Survey). Quality Level B (QLB): Utility information derived by marking the approximate surface horizontal location of utility using electronic methods. Marking is subsequently field survey located and accurately reduced onto these drawings 2. Geotechnical investigation can be offered as an optional service at each of the three sites, but fees are not included at this time (estimated at $2,500) This optional service would be at the proposed vault sites of each interconnect, and include a report that discusses the following. a. Soil boring logs and classifications; b Existing groundwater levels and estimated seasonal high levels; c Excavation and backfill recommendations; and, d. Other concerns as appropriate. TASK 3—Preparation of Construction Documents 1. Prepare Project construction drawings to include required Plan and Profile views and necessary Construction Details and Notes. Prepare the technical specifications for the project manual. The construction documents shall be complete and meet A-2 East-West Interconnect Scope(23-Sept-2013),docx requirements for construction contract competitive bid formulation and subsequent construction of the Project. Documents shall comply with current City Standards. Provide a quality assurance and "constructability" review prior to submittals to the City. The ENGINEER will submit three (3) sets of construction documents to the City for review and comment at the 90% complete level. The construction documents include the plans, specifications probable estimate of construction costs and a QA/QC certification. Meet with the City to review and discuss the 90% submittal. Following the review, revise the drawings accordingly. Included in this task are the services of an Electrical Engineer subconsultant to perform the electrical and instrumentation design for each of the three sites. 2. Submit three (3) sets of construction drawings and technical specifications at a 100% level of completion to the City. Included with submittal electronic drawing files and technical specifications. The drawing files will be submitted in AutoCADTM .dwg format, and the technical specifications will be submitted in Microsoft Wore format. 3 Submit the Engineers Estimate of Probable Construction Cost for the project based on the 100% plan set. TASK 4 -Permitting 1 Prepare and submit specific permit application and supporting documentation necessary to the Florida Department of Environmental Protection. Interconnection site right-of-ways (ROWs) are assumed to be within the City of Clermont, therefore Florida Department of Transportation (FDOT) Permits not included at this time. If FDOT permits are deemed necessary, CPH can provide that service as an additional fee Permit fees will be paid for by the City. 2. Respond to a request for additional information from FDEP. TASK 5—Bidding Assistance 1. Provide one (1) pdf copy of the Contract Documents (engineering drawings and specifications) for distnbution to potential bidders by the City's Purchasing Department. 2. Attend a pre-bid conference, prepare minutes of the pre-bid conference and submit to the City. 3. Consider written questions from bidders related to the project and prepare addenda as required to interpret, clarify or expand the Biding Documents The addenda shall be submitted to the City for distribution in a timely manner that allows reception of the addenda by bidders no later than a minimum of five (5) days prior to bid opening date. 4. Review and evaluate the apparent low bidder's unit prices, prepare a tabulation of bids, review the three (3) low bidders and verify bidder's expenence and references and make recommendations to the City regarding the award of the construction contract. A-3 East-West Interconnect Scope(23-Sept-20I31docx TASK 6—Construction Administration CPH will provide general construction administration for the interconnections that include provisions of consultation and advice Instructions to the Contractor(s) shall be issued through the City. The following tasks will be accomplished during the construction phase. The construction phase shall initiate the day after the City Commission has awarded the construction contract 1. Modify bidding documents, if required, and obtain City required and contractor executed documents, provide the City two (2) sets of the "Conformed" construction drawings and complete Project Manuals (the Contract Documents) for the City's use during the construction phase of the Project. Provide one (1) CD of PDF copies of conformed documents for the contractor's use. 2. Attend a pre-construction conference; distribute Conformed Contract Documents for the interconnection project, take meeting minutes as necessary as well as coordinate and distribute wntten minutes to attendees. 3, Review shop drawings and product submittals for conformance with the Contract Documents. 4. Conduct up to six (6) site visits at four (4) hours per visit for the project for the purpose of determining if the work is proceeding in accordance with the construction permit and approved plans and specifications. 5. Provide clarification and interpretation of the Contract Documents when requested 6. Review contractor's application for payment and advise the city of recommended payment to contractor 7. Conduct substantial and final completion inspections of Project and prepare appropriate "punch lists". 8. Prepare and submit certifications of construction completion of the project for clearance by FDEP. 9. Prepare record drawings based on the contractor's red-line markups and surveyed information. A-4 East-West Interconnect Scope(23-Sept-20I3).dotx COMPENSATION: Compensation paid to the ENGINEER for the services described herein shall be made in accordance with the Continuing Services Agreement dated October 20, 2009 between the City of Clermont and CPH, Inc. Tasks 2, 4, and 6 include the associated fees for CPH's subconsultant for Utility Designation (Task 2) and Electrical Engineering (Task 4 and 6) The total lump sum fee is $42,550.00,with each Task broken out as follows. • TASK 1 - Preliminary Engineering - $3,500.00 • TASK 2—Survey and Utility Designation - $7,050.00 • TASK 3—Preparation of Construction Documents - $16,500.00 • TASK 4—FDEP Permitting - $3,500.00 • TASK 5—Bidding Assistance - $2,500.00 • TASK 6—Construction Administration - $9,500.00 TOTAL FEE - $42.550.00 AUTHORIZATION: IN WITNESS WHEREOF, the parties hereto have made and executed this Proposal the day and year first above written.
