2009-54 10 ATTACHMENT A SCOPE OF SERVICES CITY OF CLERMONT,FLORIDA EASTERN SERVICE AREA ELEVATED WATER STORAGE TANK EVALUATION, REPAIR(S) AND COATING I. PROJECT OVERVIEW This project includes professional services for inspection and evaluation, preparation of bidding, contractual, and technical documents for repair(s) and coating of the City of Clermont's (City's) 400,000 gallon Eastern Service Area Elevated Water Storage Tank. Initially the interior and exterior of the tank will be field inspected to include evaluation of the existing coatings, accessories and structural components in accordance with guidelines of the American Water Works Association (AWWA), and Occupational Safety and Health Administration (OSHA). Subsequent to the inspection and evaluation, bidding and contract documents, engineering drawings (as applicable), and technical specifications will be prepared for any structural repair work and coating of the tank and the project advertised for competitive bids. Design Team Tetra Tech proposes the following design team to perform the various elements of the proposed Scope of Services: 1. Project Management, Bidding and Contract - Tetra Tech, Inc. Documents, and Construction Administration Services 2. Coating/Structural Tank Evaluation, Repair Design, - Tank Engineering and Manage- Drawings and Technical Specifications, Construction ment Consultants, Inc (TEAM) Shop Drawings Submittal Reviews, and Onsite Construction Observation H. SCOPE OF SERVICES A. Tank Evaluation Field Inspection The field evaluation will include the inspection of the interior and exterior of the tank b) technicians to identify sanitary, safety and structural deficiencies. The tank interior wil be pressure washed to remove all sludge and sediment. Measurements of tanl accessories, metal loss, ultrasonic steel thickness, coating adhesion, and coating thicknes: will be taken. Structural deficiencies will be noted and the existing coating will IN evaluated. The exposed portions of the foundation will be visually evaluated to locat( cracks, spalling and other types of deterioration. Field data collected will be reviewed b) a structural engineer to determine the integrity of the tank. Coating samples will be taker and adhesion tests performed to determine recommendations for painting an( rehabilitation. The cathodic protection system of the tank will be visually inspected. B; reviewing corrosion patterns and metal loss, the apparent working order of a cathodi■ protection system will be determined. Prior to inspection,the City will: 1. Isolate the tank from the system and completely drain the tank. 2. Opening of all manholes for tank access. 3. Disinfect and bacteriologically clear the tank following inspection. Coating Evaluation Coating samples will be taken during the field inspection and tested to determine thei lead, chromium, and cadmium content. The coating thickness and adhesion tests will bi done to determine the present condition of the interior and exterior coatings. Based of the test results, the existing condition of the tank surface will determine the amount o surface preparation required for painting of the tank. Structural Evaluation The field evaluation will identify any observed structural deficiencies or damage that may have occurred since the tank was constructed. Any deficiencies found will be analyzed for their affect on the structural integrity of the tank. Evaluation Report An evaluation report will be provided for the tank and include the following: 1) Observation: The observation section of the report will provide dimensions of the tank and accessories, as well as a narrative description of the condition of each part of the tank. Safety and structural deficiencies observed will be described and an assessment of the coating condition. 2) Recommendation: The recommendations section of the report will include repairs and modifications required to bring the tank into compliance with present AWWA D100, D102 and Manual M42 and OSHA safety standards. Coating recommendations will be made. When practical, alternative methods of repair, modification,and painting will be provided. 3) Economic Factors: The economic factors section of the report provides budget estimates for all of the repairs, modifications, and coating requirements outlined in the recommendations section of the report. 4) Photographs: The report will contain color photographs of the tank. The photographs serve as additional documentation of the tank's condition. 5) Summary Report: Prepare a summary report presenting the findings, conclusions, and recommendations resulting from the tasks above. The report will be certified by a professional engineer. B. Final Design Final design will result in preparation of the bidding and contract documents,engineering drawings and technical specifications for the recommended repairs and coating of the Eastern Service Area Elevated Water Storage Tank. The final design documents will be submitted to the City for review at the 90-percent completion level. A review meeting will be held with the City following the 90-percent submission. Three (3) sets of documents will be provided to the City at the 90-percent and final submittals. Tasks to be completed during this phase are summarized below: 1) Prepare a comprehensive project manual that contains bidding and contractual documents (Divisions 0 and 1) and technical specifications for competitive bidding (Divisions 2 through 16). The project manual and its contents will be formatted in accordance with the Construction Specification Institute(CSI). 2) Drawings or details, if any, to further define existing conditions, modifications/repairs and requirements of the project. 3) Final Opinion of project cost to be submitted with the 90-percent submittal. C. Bidding Upon advertisement of the project by the City, Tetra Tech will assist the City with bidding of the project and shall complete the following tasks: 1) Provide one (1) copy of the Contract Documents (engineering drawings and specifications) and any addenda, which may be issued to bidding service agencies, such as Dodge Reports. Additional sets required for bidding will be sold to bidders or others for a nominal fee approximately equal to the cost of printing and handling. The City will be responsible for advertisement of the project. 2) Attend and conduct one (1) pre-bid meeting with representatives of the City, the project team and potential bidders. 3) Respond to questions and prepare and issue addenda as required to interpret, clarify or expand the bidding documents. 4) Review and evaluate the apparent low bidder's qualifications for undertaking the work and make recommendations of the award of the contract. 5) Assist the City in obtaining the insurance, performance bond and payment bond certificates and other such submittal items required from the Contractor by the contract documents so that construction of the project can commence. 6) Prepare three (3) copies of a conformed set of Contract Documents for the City and one(1) reproducible set for the recommended Contractor. D. Construction Administration Upon award of the project, Tetra Tech will assist the City with construction administration and shall complete the following tasks based on construction duration of two(2)months: 1) Prepare for and administer one(1)preconstruction meeting. 2) Attend one(1) site progress meeting. 3) Review shop drawings to determine conformance with the design concepts of the project and compliance with the requirements provided in the contract documents. 4) Review requests for information (RFIs), provide interpretation of construction documents, and issue written clarifications or interpretations. 5) Review the Contractor's Application for Payment and the accompanying data and schedules, determine the amounts owed to the Contractor, and advise the City of the recommended payments to the Contractor. 6) Upon written request by the Contractor, conduct a substantial completion inspection of the project with the City and distribute a punch list of observed deficiencies to be completed by the Contractor prior to the final completion date. The project will be certified substantially complete only if the work is sufficiently complete in accordance with the contract documents, so that the work can be utilized for the purposes for which it is intended. The City will perform a final completion inspection to verify completion of the items noted on the punch list. E. Onsite Construction Observation Tetra Tech's tank subconsultant, TEAM, will provide inspection services during construction to observe the progress and quality of the construction work and to determine in general if it is proceeding in accordance with the Contract Documents. Onsite services will include the following: 1) Tank Repairs: Inspection of all repairs, modifications, upgrades, etc. by an inspector experienced in welding and tank construction to assure conformance of the work with the repair requirements of the contract. Inspections will be performed once per week, unless otherwise required. The inspector will maintain a log of written inspection reports. Based on the current inspection reports previously performed, it is anticipated that two (2) inspections will be required of the repair work performed. 2) Tank Coating: Inspection during surface preparation and coating operation will be provided by a NACE "trained" inspector to assure conformance of the work with the coating requirements of the contract. Inspections will be performed at the following construction progress milestones: Inspection No. Milestone 1. Beginning of Abrasive Blast and Surface Preparation 2. During Intermediate Coating Application 3 Final Coating Application Inspection with tank interior holiday testing 4. Miscellaneous inspection, if needed The inspector will maintain a log of written inspection reports. III. COMPENSATION SUMMARY The total lump sum compensation for the Scope of Services described in Section II is summarized below. A. Tank Evaluation $5,400 B. Final Design $1 1,500 C. Bidding $4,400 D.
