Vision Care, Inc. (dba) VisionCare Plan agrees to renew your vision care plan with no change in the
existing benefits for an additional twenty-four (24) months at the current rates, effective April 1, 2002,
with the acceptance of the enclosed renewal contract.
In 2000, Vision Care, Inc. was acquired by CompBenefits, a leading dental and vision benefit provider.
Your non-network benefits are now underwritten by CompBenefits Insurance Company and network
benefits are underwritten by Vision Care, Inc. VisionCare Plan, your comprehensive vision care program,
will continue to be administered by Vision Care, Inc. As such, a revised policy is enclosed for your review
and acceptance. Please retain this contract original for your records.
