Dear Dr. Spainhour:
We have reviewed the amended subcontract agreement with Clermont Police Department not to exceed
the amount of $7,500 to provide TraCS with an all-inclusive Criminal Justice Information Services
compliant disaster recovery data hosting site and services between the dates of October 1, 2020 — March
31, 2021. This subcontract is hereby approved.
The subcontract agreement will be effective the date the last party signs the agreement, and only those
charges incurred from that date will be eligible for reimbursement.
Please send us a copy of the executed subcontract after it has been signed. We will not be able to process
invoices for the services under this subcontract until we receive a copy of the executed subcontract.
A copy of this letter and the executed subcontract will become a part of your permanent project file.

Re: Request for authorization to amend existing data hosting contract.
Dear Mr. Craig:
On behalf of the TraCS Florida Project, I am requesting approval to amend the existing data hosting contract
between the Clermont Police Department and the Florida State University's TraCS Support Enhancement, and
Training (Project M3DA-2021-00129). Existing plans to migrate to a different data hosting provider have been
postponed; therefore, it is necessary to renew the contract beyond 3 months with a contract ending date of
December 31, 2020 to 6 months with a new contract ending date of March 31, 2021.

AMENDMENT TO DATA CENTER HOSTING AGREEMENT TO EXTEND EXPIRATION DATE
In this contract between Clermont Police Department (CPD) and Florida State University (Customer), the
CPD agrees to an extension of Data Hosting Services. This is an amendment to the original contract.
Details of cost and dates of service are included below.
Dates of Service and Costs
This extension agreement is made and entered into as of December 15, 2020, by CPD and Customer.
WHEREAS, CPD and Customer entered into a contract for data hosting services for 3 months, beginning
on October 1, 2020 and ending on December 31,2020, not to exceed a price of $3,750.
NOW, THEREFORE, both parties have agreed to extend the contract for data hosting services to 6
months, with a new ending date of March 31, 2021, not to exceed a price of $7,500. If Customer does
not renew the contract, the Customer's right to access the data hosting services will be terminated.
THIS AGREEMENT IS A LEGALLY BINDING AGREEMENT BETWEEN CPD AND CUSTOMER. BY SIGNING
BELOW, THE CUSTOMER WARRANTS TO CPD THAT HE/SHE IS OF EIGHTEEN (18) YEARS OF AGE OR
OLDER, IS AUTHORIZED TO OBTAIN SERVICES, AND INDICATES THAT YOU, THE CUSTOMER, HAVE
READ THIS DATA CENTER HOSTIG AGREEMENT IN ITS ENTIRETY, THAT YOU UNDERSTAND IT, AND
THAT YOU CONSENT TO BE BOUND BY ALL OF ITS TERMS AND CONDITIONS, WHERE THIS
AGREEMENT SETS FORTH YOUR RIGHTS AND OBLIGATIONS WITH RESPECT TO YOUR SUBSCRIPTION
TO THE CPD SERVICES THAT YOU HAVE SELECTED, AND YOUR USE OF AND ACCESS TO ANY SERVICE
PROVIDED BY CPD.
The parties to this subcontract shall be bound by the additional provisions in the attached
Appendix: FY2021 - Subcontract Agreement Required Federal Clauses, Per Part V.
