1999-19 . . CITY OF CLERMONT, FLORIDA SAFETY FOOTWEAR PURCHASE PROGRAM ELIGIBILITY This program shall be available for any employee who is employed in a position which requires safety footwear to be worn. The Department Head is to determine, based on criteria outlined in the City's Safety Policy which positions require safety footwear. METHOD OF PURCHASE Eligible employees will be able to purchase OSHA approved footwear from a City approved footwear vendor. Footwear purchases costing $75.00 or less shall be paid directly by the City to the vendor. The cost of any footwear chosen above this amount shall be the responsibility of the employee and the excess amount shall be paid directly to the vendor at the time of purchase. All other employees shall be able to purchase footwear from the approved vendor with the full cost being paid directly to the vendor by the employee. FOOTWEAR REPLACEMENTS The City will purchase replacement footwear for eligible employees once annually, unless a job related incident occurs which affects the protective capability of the footwear. PROBATIONARY EMPLOYEES In the event an eligible employee who has received footwear in accordance with this program terminates prior to successfully completing probation, the full cost of the footwear provided by the City shall be deducted from the employees final pay check.
