SECOND AMENDMENT TO AGREEMENT FOR
PUBLIC SAFETY PHYSICAL EXAMINATION SERVICES
THIS AMENDMENT is entered into as of this 25t' day of August 2022 and is to that
certain Agreement dated August 27, 2019, No. 2019-66 hereinafter "the Agreement", by and
between the CITY OF CLERMONT, FLORIDA, a municipal corporation under the laws of the
State of Florida whose address is: 685 W. Montrose Street, Clermont, Florida, hereinafter referred
to as "CITY' and FLORIDA AND LIFE EXTENSION CLINICS, INC. d/b/a LIFE SCAN
WELLNESS CENTERS, 1011 North MacDill Ave., Tampa, FL 33607 hereinafter referred to as
"CONTRACTOR". The parties, in exchange for the mutual covenants contained herein and in the
Agreement, agree as follows:
1. This Amendment expressly modifies the Agreement and in the event of a conflict,
the terms and conditions of this Amendment shall prevail.
2. The Compensation Schedule is being amended as provided in Exhibit A.
3. All other terms and conditions set forth in the Agreement shall remain in full force
and effect and unchanged as agreed to by the parties.

Re: 2nd Contract Renewal Notice for Medical Examination and Drug/Alcohol Testing Services, No.
2019-66
Dear Patricia Johnson:
The current awarded period on the above referenced contract is due to expire on August 26, 2022. There
is a provision for an extension in this contract for up to four (4) additional one (1) year terms. It is the intent
of the City to exercise the second renewal option and extend the contract for an additional 1-year, ending
on August 26, 2023.
Please check "yes" or "no" at the bottom of this letter, with your signature and date, as to whether you are
interested or not extending the contract for an additional 1-year term. Your response should be received
in the Procurement Services Department no later than ten (10) business days from date of this letter.
Failure to respond will be deemed as silent evidence that your firm is not interested in renewing this
contract. Make sure to send a copy of your insurance certificate stating the City of Clermont as additional
insured to agates(@clermontfl.org
