COMMUNITY SERVICE PROGRAM SUPERVISING AGENCY AGREEMENT
CITY OF CLERMONT - CITY HALL , hereinafter referred
to as supervising agency, and The Lake County Probation Office mutually
covenant and agree to a program of Community Service.
The Supervising Agency agrees to the following conditions:
1.
Provide work for the participant and provide necessary
working materials.
2.
Provide safe working cooditions.
3.
Will not require participaot to perform any duties that may
be hazardous.
4.
Provide normal supervision of work of participants.
5.
Notify the probation office of any participant who fails to
show after beiog assigoed.
6.
Return Community Service Assignment sheet, signed and dated,
with completed work hours written on it.
Lake CounLy Probation Office agrees to the following conditions:

1.
Provide appropriate participants whenever possiþle to
supervising agency.
2.
Notify supervising agency of any changes of participaots to
report to that agency.
3.
Promptly and effectively handle any problems that may arise
in the Program.
