This is a service agreement between Chlorinator
Services, Inc. (CSI) and the undersigned. It will
remain in effect for one (1) year from date of issue
and provides a guarantee that the specified hourly
charges and the specified mileage charges will not
increase while this agreement is in effect.
Billing to be made at A'/-:!!!-_. per hour and ; 3(] per mile.
