1972-03Addressograph Multlgraph CorporatIon ("COMPANY") PREVENTIVE MAINTENANCE AGREEMENT The Company agrees to perform preventive maintenance service and furnish necessary replacement parts on the Addressograph Multigraph equipment listed by model and serial number and for the prepaid fees shown on the reverse hereof, subject to the following: 1. The equipment will be cleaned, lubricated and adjusted periodically, at the frequency shown on the reverse hereof. Any necessary suggestions to the operator will also be furnished at the time service is performed. The cleaning referred to herein, to be performed by the Addressograph Multigraph Corporation Service Representa- tives, is cleaning necessary to keep the equipment in good, efficient mechanical condition. Cleaning which is necessary to keep the equipment in good operating condition, between regularly scheduled maintenance calls, is the responsibility of the operator(s) of the equipment. 2. This Agreement includes replacement of STANDARD PARTS necessary to maintain said equipment in good condition and LABOR necessary to make such replacement of parts, technical adjustments, cleaning and lubri- cating. Exposure lamps for copiers are also included. 3. This Agreement includes "special calls" (service calls made between the regularly scheduled preventive mainte- nance calls) requested by the customer and found by the service representative to be necessary to keep equipment in good condition. 4. This Agreement includes all travel expense EXCEPT on special service calls requested by customers located more than 25 miles from source of service, in which case a charge will be made for travel expense only (not for labor). 5. This Agreement includes labor for replacement of motors, but does not include the cost of motors replaced due to changes in power line specifications. 6. Labor, parts and expense necessary to repair damage caused by accident, fire, water, or necessary for major repairs, overhauling or altering equipment are not included in this Agreement. When such service is required, an estimate of charges will be submitted for approval before work is started. 7. This Agreement does not include new attachments or operating supplies such as: For Addressograph equipment-ribbons, ink, plates, frames, nickel-types, or parts and sharpening on Addresso- graph 1977 and Series 7200 and 9000 die and punch units, Type Di sc for 320 Graphotype. For Copier and Duplicator equipment-Multi lith blankets, ribbons, ink, ink rolls (soft rubber, felt or compo- sition), felt wicks, sponges, blanket cleaning roller, rubber rolls for continuous load feeders, rubber Repelex rolls, molleton covers nor labor to replace molleton covers, Curastat Bars and transformers for Static Eliminators, rubber platens, electrotypes, numbering heads, knives, knife sharpening, cutter sticks, paper, toner, replenisher, all glass on Photo-Direct equipment, and Copy Guide and Copy Board Glass on 805. 8. Installation of new ink rolls, Repelex rolls, ribbons, blankets, cutter sticks, knives for cutters and restringing of Corona and bias assemblies will be performed without charge if the service representative is present for other reasons. A regular service charge will be made if a special call is requested only for the purpose of installing these items. 9. Any new attachment added to a machine covered by thi s Agreement will be maintained for the additional fee, if any, as shown on the Company's schedule of rates. Machines used for multiple shift operation are subject to an extra charge. .' . . . . 10. It is understood that the equipment covered by thiS Agreement IS In good condition on the date thiS Agreement becomes effective. All service under this Agreement will be performed on the customer's premises during regular business hours of Company's sales office. If service outside of such hours is requested, service will be rendered at Company's regular hourly rates plus 50%, subject to availability of service personnel. 11. This Agreement shall remain in effect until canceled by either party on 30 days advance written notice to the Branch rendering the service, provided that after the end of any prepaid period, the continuance of this Agreement shall be at Company's then current rates. Such cancellation may be effected as to the entire Agreement or as to anyone or more of the machines covered hereby. Upon such cancellation prior to the end of any prepaid period, Customer will be refunded the pro-rata amount based on the number of regularly scheduled service calls paid for but unperformed. . 12. Fees plus applicable taxes payable in advance at billing frequency shown on reverse hereof. Terms, net on receipt of invoice.
