You are directed make the changes noted below in the subject contract:
NATURE OF THE CHANGES:
Refer to the attached summary of changes (Attachment A).
Purpose of this Change Order:
Decrease the Contract Price to reflect cost reimbursement from the Contractor for services not
provided:
- Elimination of multiple Stainless Steel Valve Stem Extensions
- Elimination of the Handrail from the edge to the center of the GST dome.
- Substitution of Tamms Ultra Kit in lieu of Thoroseal on the Ground Storage Tank.
Increase the Contract Price to reflect additional costs associated with the following project
modifications:
- Install WIN-911 Alarm Notification System at the Sunburst Lane WTF.
- Provide two (2) each 1,150 gallon capacity Hypochlorite storage tanks
o Chem Feed Additions
- Modify the roofing to install Ridge Roof Vents
- Provide fire extinguishers
- Provide Duct liner in the HVAC System
Provide wall hydrants
- Install Eyewash Stations
